We're delighted to warmly welcome all new families joining our school community. 

The Sag Harbor School District in partnership with all members of the community is committed to equity in education; its mission is to provide students with a safe, child-centered environment which fosters personal, academic, and creative excellence. Our dedicated team is here to assist you every step of the way as you settle into your new educational home.

  1. HOUSING QUESTIONNAIRE
  2. PARENT/GUARDIAN PHOTO IDENTIFICATION – The parent/guardian of the child must be in attendance at the time of registration. The child doesn't need to be present at the time of registration. The parent/guardian will need photo identification to access the building.
  3. STUDENT PROOF OF BIRTH – Either Birth Certificate or Passport An original copy is required. A copy will be made by the registrar.
  4. RESIDENCY VERIFICATION – To verify residency at the time of registration, the following is a non-exhaustive list of acceptable documentation.
    • HOMEOWNERS – You MUST present the following documents: Real Property Tax Bill or Signed Deed AND Two (2) of the following current documents in the Homeowner’s name: Mortgage Statement, Utility Bill, Property Insurance Certificate, Voter Registration, Recent W2 Form

    • RENTERS – You MUST present the following documents: A valid and fully executed lease AND Two (2) of the following current documents in the Renter’s name: Utility Bill, Voter Registration, Recent W2 Form, Renter’s Insurance Certificate

  5. NEW YORK STATE HOME LANGUAGE QUESTIONNAIRE – To provide your child with the best possible education, we need to determine how well he or she understands, speaks, reads, and writes in English, as well as prior school and personal history. Follow the link on our website to complete the form in your dominant language.

  6. IMMUNIZATIONS – Proof of required immunizations for school entry is mandated by the New York State Department of Health and must be presented at the time of registration. Proof must be on a physician’s letterhead and signed by either a physical signature, electronic signature, or signature stamp. Current immunization requirements can be found on our website.
  7. HEALTH EXAMINATION – The New York Department of Health requires all new entrants to have a Health Examination. The physical examination form will be accepted if it is dated not more than twelve months before the student’s first day of attendance.
  8. NEW STUDENT RELEASE OF INFORMATION FORM –  Follow the link on our website to complete the release of information form.
  9. CUSTODIAL DOCUMENTS (if applicable)
