STUDENT REGISTRATION PACKET
Welcome to the Sag Harbor School District.
Registration Hours are by appointment only.
Please review all information and questions in this packet. Should you have any questions about the forms or documents that are required, please email at registration@sagharborschools.org.
Children must be four (4) years of age on or before December 1 in order to be enrolled in pre-kindergarten for the coming year.
Please complete all of the forms as indicated.
Please supply all necessary documents as indicated.
Please note that some forms may require certification by a Notary Public.
When the forms are completed and you have compiled all the necessary documents, please submit them to:

In order to help you complete the paperwork in a timely fashion, to verify residency at time of registration, the following is a non-exhaustive list of acceptable documentation.
FOR HOMEOWNERS:
You MUST present the following documents:
Real Property Tax Bill
OR
Signed Deed
AND Two (2) of the following current documents in the Homeowner’s name:
Mortgage Statement
Utility Bill
Fuel Oil Bill
Cable TV Bill
Voter Registration
Property Insurance Certificate
Recent W2 Form
FOR RENTERS:
You MUST present the following documents:
Two (2) of the following current documents in the Renter’s name:
Utility Bill
Recent W2 Form
Fuel Oil Bill
Cable TV Bill
Voter Registration
Property Insurance Certificate
AND either 1 or 2 below:
A valid and fully executed lease for the rental unit and a canceled check or rent receipt signed by the landlord, including the landlord’s address, telephone number and property address and a completed, signed and notarized “Affidavit of Renter/Tenant”
A completed, signed and notarized “Affidavit of Property Owner/Landlord”
and a completed, signed and notarized “Affidavit of Renter/Tenant”
